 If I haven't had it tuned since Grandma bought it new in 1925, will I have to pay extra for bringing it up to concert pitch?

Since everybody else has thrown in, I'll put my $.02 in. Ya'll make it too hard on yourselves. Since the guitar is tuned to "E".(concert G) You will need to tune the E's to 440.tune the beats out of your 3rds and 5ths. Then it will be time to tune your pedals. "A" pedal first, then the "B" pedal. Then tune your levers. Then go back to your "C" pedal, and your "Franklin 4rth" if you have it installed on your axe. If you have a problem hearing the "beats" you can crank up as much distortion on your amp as you can, and it will make the beats stand out.(an old rock and roll guitar trick.)By keeping your "E"s in pitch, you'll be in pitch with the rest of the tune freak out of pitch tele players, and also the keyboard player. Different string sets will screw you up too, even if they are the same gauges. Different aloys will get you too. Just think, you've got nickels on your axe, you bust one in the second set, so you grab a spare outta your seat, and it's a leftover stainless. That G# won't be in pitch the rest of the night.
